  • Registered Nurse

    Full-time, part-time or per diem Registered Nurse with home health experience. Must have at least one year of medical/surgical and/or critical care nursing experience, case management experience in home care is a plus, have a current California RN license, willing to visit patients in Fresno and surrounding counties. RN must have excellent oral and written communication skills, ability to interact effectively with all staff, patients, families and members of the health team, effective problem-solving skill and good analytical skills. The RN collaborates with the interdisciplinary team. Current CPR certification. Position requires driving.

  • Physical Therapist

    Physical Therapist to provide physical therapy to patients of all age groups. The Physical Therapist plans, organizes, and conducts physical therapy treatment program based on the medical referral and their evaluation. The Physical Therapist plans, implements, and evaluates patient care plans to restore or maintain patient wellbeing. The Physical Therapist collaborates with the interdisciplinary team. One-two years experience as a Physical Therapist in an acute care or rehabilitation setting, previous home care experience preferred. Current CPR certification. Position requires driving.

  • Occupational Therapist

    The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. The Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient’s wellbeing, and evaluates the patient’s progress. The Occupational Therapist collaborates with the interdisciplinary team. One-two years clinical experience as an Occupational Therapist. Current CPR certification. Position requires driving.

  • Home Health Aide

    The Certified Home Health Aide performs various personal care services, observes patients, reports observations, documents observations, and care performed under the supervision of a registered nurse or therapist. The Home Health Aide will be assigned in a manner that promotes quality, continuity, and safety of a patient’s care. Current Home Health Aide and CPR certification. Position requires driving.

  • Speech Therapist

    We have opportunities to work with the full lifespan. Whether you are interested in working with children or adults, we offer you the opportunity to design your caseload based around your preferences. Minimum one year Speech Therapy experience preferred. Experience with Dysphagia and Aphasia patients preferred. Home Health experience is a plus. Current and unrestricted Speech Language Pathology licensure, current CPR certification. Position requires driving.

  • Licensed Vocational Nurse

    Responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Current LVN licensure and CPR certification. Position requires driving.
